At Capital Eye Consultants we have more state-of-the-art diagnostic equipment than most ophthalmic practices in the Washington, D.C. area.  This allows us to provide “in-house” testing that assists in making accurate and efficient diagnoses. Appointments

We see patients by referrals and appointments only.  If you are unable to keep an appointment, 24-hour notice is requested. Missed appointments are subject to a $40.00 fee.  If you are more than 15 minutes late and fail to notify our office, your appointment will be subject to cancellation and we will ask to reschedule your appointment. 

If you are visiting our office for the first time, please arrive 10 minutes early to complete the check-in process if you have completed our forms in advance (see forms above).  Bring those forms with you as well as a photo ID and insurance card(s).

Please arrive 20 minutes early to complete the check-in process and fill out the required documents in our office if you are unable to do so beforehand.  Established patients will be asked to update their forms and insurance information annually.

Failure to provide us with an insurance card and a current insurance referral (if required, see Insurances Accepted below) could result in your not being seen by the doctor.

We are in-network with many insurance carriers. If you do not see your insurance on this list, please call our office.

Some insurance plans require an insurance referral from your primary care physician as seen in most HMO (health maintenance organization) plans.  It is your responsibility to secure the insurance referral from your primary care physician.  You can have your referral faxed to us at 703-876-0163 or email to referrals@ceceyes.com. You may bring it with you to your appointment, but it is helpful to have the insurance referral beforehand so we can verify the physician’s office has, in fact, completed the form correctly.  

If you have an insurance that our office participates with, we will submit your claim for you. Your co-payment is due at the time of your service.

If we do not participate with your insurance plan, payment in full is expected at the time services are rendered.  For your convenience, Visa, MasterCard, Discover, American Express, personal checks and cash are accepted.

HIPAA Privacy and Medical Records Release

By law, we are required to provide you a detailed document about HIPAA patient privacy rights. Our Capital Eye Consultants’ Disclosure of Health Information form must be completed and signed acknowledging you have received this information.

Please note that HIPAA regulations allow only you (the patient) to sign to have your records released to yourself (fees apply), your designated representative (fees apply), or another physician (no fee). Charges for copying medical records are in compliance with the State of Virginia medical record copying guidelines. Payment is requested prior to the release of your records.
